Eosinophilic Chronic Rhinosinusitis

aKBB AD, Gazi Üniversitesi Tıp Fakültesi, ANKARA Chronic rhinosinusitis represents a challenge with its poorly understood pathophysiology and limited treatment options. The disease is characterized by a broad diagnostic and etiologic spectrum. The presence of eosinophilia histopathologically in chronic rhinosinusitis is frequently associated with greater disease and a decreased likelihood of surgical success.
Eosinophilic chronic rhinosinusitis encompasses a wide variety of etiologies and associations that can be grouped under this umbrella term. Several classification schemes and diagnostic criteria describe chronic rhinosinusitis and make comparisons difficult. Recent findings supporting mechanisms that promote eosinophilic infiltration are discussed and include the following subcategories: super antigen-induced eosinophilic chronic rhinosinusitis, allergic fungal sinusitis, nonallergic fungal eosinophilic chronic rhinosinusitis, and aspirin-exacerbated eosinophilic chronic rhinosinusitis.
Although steroids remain the nonspecific hammer in the therapy of all these eosinophilic conditions, improved understanding of the pathophysiologic basis of the particular form of eosinophilic chronic rhinosinusitis provides a hope that more targeted therapy will be equally efficacious without the cost of steroid morbidity.Keywords: Eosinophilic rhinosinusitis, nasal polyp, chronic rhinosinusitis, superantigen, allergic fungal sinusitis, categorization of chronic sinusitis, fungi, eosinophils, inflammationTurkiye Klinikleri J Surg Med Sci 2005, 1(3):21-25
