Surgical Indications And Procedures In Rhinosinusitis
Dr. Kemal DEĞER,a Dr. Tayfun DEMİRELa
aKBB AD, İstanbul Üniversitesi İstanbul Tıp Fakültesi, İSTANBUL Chronic rhinosnusitis is one of the most frequent otolaryngologic diseases encountered. Endoscopic diagnostic examinations in conjuction with modern imaging methods, particularly computed tomography (CT), have proven to be an ideal combination in recent years and are accepted as the standart care in many parts of the world. As a logical consequence of diagnostic findings, an endoscopic-surgical concept has resulted which is directed at the diseased areas in key positions of the lateral nasal wall. Using less traumatic interventions which preserve the mucosa, a cure was possible in the majority of all cases of chronic sinusitis, without having to resort to the more radical surgical methods. For chronic sinusitis today, interventions which used to be directed at the large sinuses, are targeted today at their prechaimbers in the lateral nasal wall: the ostiomeatal complex with the narrow infundibulum of the ethmoid and the frontal reses as key positions. Endoscopic sinus surgery requires detailed knowledge of paranasal sinus anatomy. This article mentions about the surgical anatomy of the sinuses, anatomic variations, and key surgical landmarks and surgical technique of endoscopic sinus surgery which is the principal surgical therapy in chronic sinusitis, accepted worldwide. Keywords: Chronic rhinosinusitis, endoscopic sinus surgeryTurkiye Klinikleri J Surg Med Sci 2005, 1(3):39-46
