Adhesive Otitis Media

aKBB AD, Ege Üniversitesi Tıp Fakültesi, İZMİR Adhesive otitis media is one of the sequela of otitis media caused by a long term inflammation which ends up with an intact tympanic membrane that is adhesive to the promontorium and ossicles. Although it is used as a synonym with atelactasis, it would be better to evaluate adhesive otitis as an end stage process of atelectasis. The basic problem is the increased negative pressure in the middle ear. Though there are many factors to blame, eustachian tube dysfunction seems to be the leading one. The disease generally begins in early childhood, and mostly accompanied by hypertrophy of adenoids, allergic conditions, and paranasal sinus enfections. There is usually a conductive type hearing loss which may be complicated by biochemical and toxic events through round window causing a cochlear type neurosensorial hearing loss. Treatment options including tympanoplasty techniques may be used but not advised since it’s almost impossible to detach the adhered membrane from middle ear mucosa. Therefore hearing aid is the best choice for the patients having bilateral hearing loss that interferes with the social communication. As a conclusion, instead of treating adhesive otitis media, it would be much better and wise to treat otitis media with effusion and atelectasis in early stage. For this purpose, one can insert a ventilation tube to ventilate the middle ear and reversing the pathologic process by replacing the eustachian tube function.
