Petrositis

aKBB AD, Ankara Üniversitesi Tıp Fakültesi, ANKARA Petrositis is a rare complication of otitis media, observed in the presence of petrous bone pneumatization, where the infection extends to the petrous apex through the mastoid cells. The symptoms are due to irritation of the surrounding structures of the petrous bone including retro-orbital pain from irritation of the trigeminal ganglion in Meckel’s cave, paralysis of the ipsilateral abducent nerve as it passes through Dorello’s canal, or dysfunction of the facial, cochlear, or vestibular nerves as they course through the internal auditory canal. The most common isolated bacteriae are Staphylococcus aureus, Streptococcus pneumonia and Streptococcus pyogenes. The diagnosis of petrositis relies mainly on high index of suspicion and imaging studies including complementary compterized tomography and magnetic resonance imaging of the temporal bone. The treatment of petrositis requires long term intravenous antibiotic coverage, decided according to bacterial culture results of the middle ear content obtained via a myringotomy and drainage surgery, if bone destruction is evident.Keywords: Petrous bone, otitis mediaTurkiye Klinikleri J Surg Med Sci 2005, 1(7):79-82
